Please do not submit Social Security numbers, account passwords, financial account numbers, or confidential medical details through website forms.

Vendors and third-party services

Google Analytics, Meta Pixel, Microsoft Advertising UET, Microsoft Clarity, heatmaps, and session replay are not enabled by default. Vendor use and BAA status must be reviewed and approved for each exact service before analytics or monitoring is enabled.
Sensitive content and analytics
Sensitive form content, names, email addresses, phone numbers, support messages, health-related intent, and other user-entered text are not intentionally sent to analytics providers. Cookie consent is not a HIPAA authorization. Any workflow that may disclose health-related or otherwise sensitive information to a vendor requires documented legal and security approval and applicable agreements before being enabled.
Consent records
When a visitor accepts, rejects, customizes, withdraws, or updates privacy preferences, the site records a minimized consent audit entry with a pseudonymous session identifier, timestamp, action, selected categories, policy version, banner version, route, locale, and coarse browser class. Raw IP addresses, form content, names, emails, phone numbers, and PHI are not intentionally stored in consent records. Current retention target: 365 days unless adjusted after legal review.
Data security and retention
We implement reasonable administrative, technical, and organizational safeguards to protect submitted information. We retain personal information and consent records for as long as necessary to fulfill operational, legal, reporting, and compliance purposes, subject to final retention schedules approved by the organization.
Your choices and rights
Depending on your jurisdiction, you may have rights to access, correct, delete, object to, or restrict processing of personal information. You can reject optional analytics or update your preferences using the Cookie and Privacy Preferences link in the website footer.
Children's privacy
This website is not intended for children under 18, and we do not knowingly collect personal information from children.
